Planning Your Visit
The Executive Physical program at Northwestern Medicine Executive Health has been designed with a full understanding of your demanding schedule. Rest assured that preparing for your annual exam takes very little of your time.
Step 1: Schedule Your Executive Physical When you are ready to schedule your executive physical, simply contact us by phone or e-mail, so that we may plan your day of care including optional specialists and testing. Step 2: Tell Us about Your Medical History Prior to your first visit, we ask you complete a medical history questionnaire (first visit only). Step 3: Pre-visit Physician Consultation You and your Northwestern Medicine Executive Health physician have a pre-visit telephone consultation to discuss your healthcare needs and concerns. Together you will collaborate on what should be included in your examination. If the physician would like to review prior medical records, we obtain those records on your behalf. Step 4: Prepare for Your Appointment Northwestern Medicine Executive Health will contact you prior to your appointment with your customized itinerary and preparation instructions.
